Starting July 1, 2026, Medicare began offering access to certain GLP-1 weight loss medications through the temporary Medicare GLP-1 Bridge program at a much more affordable rate. For eligible patients with Medicare drug coverage, the monthly pharmacy copay is $50 for a one-month supply of glp-1 medications for weight loss. This is a huge opportunity to expand access to these game-changing medications. The program is available nationwide and is currently scheduled to run through December 31, 2027 with hopeful, but not promised extension thereafter.

For patients in all of Colorado: Denver, Lakewood, Littleton, Golden, Arvada and more – this represents a major shift and expansion of access. Many Medicare patients have been paying high cash rates for GLP-1 medications, stretching doses, stopping treatment early, or assuming these medications were completely unavailable through Medicare.

The new bridge program does not mean everyone qualifies. It does not remove the need for medical supervision. It also does not replace nutrition, strength, protein, and long-term maintenance support.
But the Medicare bridge program does offer a meaningful expansion of access and affordability of glp-1 medications.

Here is what patients should know.

What is the Medicare GLP-1 Bridge program?

The Medicare GLP-1 Bridge is a temporary CMS program designed to improve access to certain GLP-1 medications for weight loss or weight maintenance.

According to Medicare.gov’s weight loss drug coverage page, covered medications currently include:

  • Foundayo tablet
  • Wegovy injection or tablet
  • Zepbound KwikPen

Who may qualify for Medicare GLP-1 coverage?

Medicare says patients must have Medicare drug coverage, such as a standalone Part D plan or a Medicare Advantage plan with drug coverage. The program is available to certain people with Medicare drug coverage, including PDP, MA-PD, Special Needs Plans, employer or union group waiver plans, and LI NET.

Eligibility is based on BMI and certain health conditions.

You may qualify if, when starting GLP-1 therapy, you are 18 or older and meet one of these criteria:

  • BMI of 35 or higher
  • BMI 30 to 34.99 with at least one qualifying condition, such as prediabetes, uncontrolled high blood pressure, stage 3a or higher chronic kidney disease, previous heart attack or stroke, symptomatic peripheral artery disease, or diastolic heart failure
  • BMI 27 to 29.99 with prediabetes, previous heart attack or stroke, or symptomatic peripheral artery disease

Medicare also states that patients are not eligible for the Bridge program if they already receive GLP-1 coverage through their Medicare Part D plan, or if they have type 2 diabetes, moderate-to-severe sleep apnea, or fatty liver disease. However, those conditions may be covered through other Medicare benefit pathways.

This is why it is important not to self-screen based only on one diagnosis. A medical provider can help you understand which pathway may apply and get you efficient access to the bridge program.

How much does the Medicare GLP-1 Bridge program cost?

Eligible patients pay a $50 copay at the pharmacy for a one-month supply, usually 28 or 30 days depending on the medication. Medicare notes that this copay does not count toward the yearly drug plan deductible or out-of-pocket limit, does not appear on the Part D Explanation of Benefits, cannot be lowered by Extra Help, and cannot be spread across months through the Medicare Prescription Payment Plan.

That means the monthly price may be predictable, but patients should still understand how the program works before assuming it behaves like regular Part D coverage.

The $50 Bridge program cost refers to the pharmacy copay only. At Weight Loss NP, we accept Medicare for covered office visits, and patients may also choose to add our optional GLP-1 Success App for extra education, accountability, nutrition support, and community.

Does the Medicare GLP-1 Bridge require prior authorization?

Yes. Medicare says the provider must send a prescription for a covered GLP-1 drug to the pharmacy and complete a prior authorization through the specific Bridge Program system. The provider must also certify that the medication is being used as part of a lifestyle program focused on diet and exercise.

This is important.

GLP-1 medications work best when they are part of a full medical weight loss plan. A good plan should include:

  • Protein and Nutrition goals
  • Strength training or muscle preservation support
  • Hydration and constipation prevention
  • Side effect management
  • Dose planning
  • Lab and medical history review when appropriate
  • Maintenance planning
  • Clear follow-up

Why medical supervision matters for Medicare patients

GLP-1 medications can be effective, but older adults need thoughtful care.

Common issues we watch for include nausea, constipation, low appetite, dehydration, muscle loss, medication interactions, low protein intake, and excessive weight loss without enough strength support.

For many Medicare patients, the goal is not just a lower number on the scale. The goal is better metabolic health, mobility, function, strength, blood pressure, blood sugar risk reduction, and quality of life.

A safe GLP-1 plan should protect muscle and function while supporting fat loss.

That means we talk about:

  • Getting enough protein, even when appetite is low
  • Eating enough nutrient-dense food
  • Preventing constipation
  • Walking and resistance training
  • Monitoring side effects early
  • Avoiding overly aggressive dose increases
  • Planning for long-term maintenance

What if you do not qualify?

Not qualifying for the Medicare GLP-1 Bridge does not always mean there are no options.

Some patients may have GLP-1 coverage through another diagnosis or their specific Part D plan. Some may be better candidates for a different weight loss medication. Some may need a hormone-informed or metabolic evaluation first. Others may benefit from nutrition coaching, maintenance support, or a lower-cost cash-pay pathway.
